Community Health Diabetes Education
Whether you have diabetes yourself or want to learn more about the condition, we offer expert diabetes advice to both patients and the local community.
Through our diabetes education service, our goal is to help those with diabetes to live as comfortably as possible.
The Diabetes Education service is provided by a specially trained nurse who has experience, qualifications and advanced knowledge in the care of people with diabetes and those at risk of diabetes.
Any adult or carer over the age of 18 with: Type 1 or Type 2 can self- refer or be referred by their doctor for an appointment.
